Health Connections Team

The Health Connections team at Frome Medical Practice is available to people who would like support with health and wellbeing issues, for example self-managing a long term health condition, increasing social connectedness or changing health behaviours.

Patients can have one to one appointments with a Health Connector, attend one of our many peer support groups, train as a Community Connector or join us at one of our Talking Cafes.
