Complex Care Team
The Complex Care Team at Frome Medical Practice offers support to patients in their communities to help manage their health care needs and ensure people are able to access the support they require to live as independently as possible.
Part of the role of the team is to follow up patients on discharge from hospital who may have long term health care needs and may require support once home.
Working closely with other health and social care providers such as District Nurses, Older People's Mental Health, Adult Social Services, Occupational Service, Physiotherapist and Mendip Health Connections we can co-ordinate care and the support required for our patients and their families thereby helping them to better manage their conditions and live more independent and empowered.
Our Complex Care Team phone line is open 10am - 5pm Monday to Friday.
