Diabetes Group Consultations
What does Frome Medical Practice offer?
Frome Medical Practice runs group consultations for diabetic patients as part of their annual review. Our GP-led group consultations provide a chance for patients to discuss their condition in more detail, share advice, and learn from each other.
Unlike traditional diabetes reviews, which usually last 10 to 15 minutes, our group consultations last about 90 minutes. Sessions are run in person, which gives you more time to talk with your healthcare team and connect with others who have similar experiences. A Health Connector will also be there to help you identify and make small, manageable changes to improve your results.
How can I take part in the sessions?
Diabetes group consultations will be booked for you by your Health Care Assistant. They will be booked during tests for your annual review.
The dates of the sessions will vary each month.
What will the consultation cover?
You’ll be welcomed to the session by a clinician and a health connector who will outline what to expect in the consultation. Then as a group you will:
- Talk through each person's results
- Have dedicated time to ask the clinician questions
- Each person is guaranteed to have one question answered but you also have the chance to learn from others questions
- The group will finish with a health connector who will support you to make small manageable changes to improve your test results.
- You will be referred to our free programme Habits for Health for further support improving your results through achievable lifestyle changes.
